What did this study measure?
We ran 21 patient-intent cost queries through Google across seven Metro Vancouver sub-markets — rhinoplasty, breast augmentation and liposuction — and recorded every source cited by the AI Overview each one returned.

- 41% pointed at Google listings, 30% at local clinic websites, 20% at clinics outside the region and 9% at non-providers
- Only 16 distinct Metro Vancouver clinics were named across the entire region
- Two consolidations applied where several domains resolve to one practice at one published address
- Single point in time — captured August 2, 2026, and generated answers vary between runs
This measures Google AI Overviews only. It does not measure ChatGPT, Perplexity, Gemini or Copilot, which use different retrieval systems and require separate tracking.
Four Findings
The local field is tiny and top-heavy. Just 16 Metro Vancouver clinics were cited across the entire region, and the top three hold 53% of local citations. Three of those — Pacific Plastic Surgery Group, Dr. Colin McInnes and YES — appear in all seven sub-markets. Below them the market thins out fast.
An Ontario clinic is the second most-cited website in Vancouver. royalcentreofplasticsurgery.com, in Barrie, Ontario, takes 18 citations across all seven sub-markets and all three procedures — more than every Metro Vancouver clinic except one. In total 20% of citations go to clinics with no Metro Vancouver location: Barrie, Montreal, Toronto, Mississauga, Ottawa, Abbotsford, Nanaimo, Ohio, Virginia — and Vancouver, Washington.
Rhinoplasty and liposuction are listing markets; breast augmentation is not. 50% of rhinoplasty citations and 51% of liposuction citations are Google Business Profile panels. Breast augmentation returns just 3% listings — and it is the one procedure where local clinics lead, at 56%.
Vancouver proper returned no listing citations at all. Zero, across all three procedures. Surrey returned 40, Burnaby 22, Richmond 22, Coquitlam 21 and Langley 20. A Business Profile is worth nothing downtown and a great deal everywhere else.

Patients travel for a surgeon, so geography barely constrains the answer. What does change, sharply, is whether Google quotes a website or a listing — and this is the sharpest split of the three cities we have measured.

Clinics From Outside the Region
17 Clinics, 62 Citations, 20% of the Market
Every domain below was opened and its published address recorded. None has a location in Metro Vancouver. salmoncreekps.com is Salmon Creek Plastic Surgery of Vancouver, Washington — about 500 km south, in a different country. It was cited in two Metro Vancouver rhinoplasty answers. Google's summariser matched the city name and not the jurisdiction; it is the same failure mode we found in Vancouver dentistry, where a Richmond Hill, Ontario practice was cited for Richmond, BC.

Vancouver Returned Zero Listing Citations; Surrey Returned Forty
In Burnaby and Richmond, out-of-region clinics are close to level with local ones. Downtown, they are not — and downtown is also the only sub-market where a Google Business Profile earns nothing at all.

82 Businesses Named Through a Google Listing
128 citations (41%) point to a Google Business Profile panel rather than a website. These carry no publisher domain, so they are reported by business name. The list is dominated by individual surgeons and medical-aesthetics clinics, many of which have no cited website anywhere in this study — Google knows them, but has nothing of theirs to quote.

Where Else the Citations Go
Financing Companies, Medical Tourism and a Deals Marketplace
28 citations (9%) went to sources that are not clinics: three patient-financing companies, two medical-tourism platforms — one based in Kyiv — a US surgeons' association, a deals marketplace and an insurer. Financing companies answer the cost question directly, which is exactly what the query asks.

Method
Proof, Not Promises
- STEP 01
Ran 21 patient-intent cost queries through Google with city-level location targeting across seven Metro Vancouver sub-markets: Vancouver, Burnaby, Richmond, Surrey, North Vancouver, Coquitlam and Langley.
- STEP 02
Tested each sub-market with the same three procedures — rhinoplasty, breast augmentation and liposuction, so every area was asked identical questions.
- STEP 03
Captured the complete search result page for each query, including the AI Overview block, its generated text, and every source it cited.
- STEP 04
De-duplicated citations per answer, so a clinic cited twice within a single AI Overview counts once.
- STEP 05
Opened every cited provider domain individually and read its published address from the clinic's own contact page. Abbotsford and Nanaimo are treated as outside Metro Vancouver and named explicitly; where several domains resolve to a single practice at one published address they are counted once and the consolidation is stated.
- STEP 06
Counted Google Business Profile panels separately as listing citations, attributed to the business named in the panel, and classified financing companies, associations, directories, medical-tourism platforms, insurers and social platforms as non-provider sources.
- STEP 07
Crawled on August 2, 2026 without personalisation, and recorded the crawl window so any figure can be re-checked against the day it was captured.
Limitations
This is a single-point-in-time snapshot. AI Overviews are generated fresh per query and vary between runs, so exact citation lists will differ if re-run; trends require repeated sampling. Clinic identification is by website domain, and where a group operates several sites each is counted separately unless the study states a merge.
Only three procedures were tested; facial, injectable and body procedures not covered here may behave differently. Several cited businesses are medical-aesthetics clinics rather than surgical practices. Two domains block automated retrieval and are reported as unverified rather than assigned to a region. Listing citations carry no publisher domain and are counted separately from website citations, so the two figures are not directly comparable. Sub-market labels follow Google's own location targeting rather than municipal boundaries.
AI Overviews reach the largest audience of any AI answer surface — no login, no app, shown to anyone searching Google — so this is the widest-reach measurement available. It is not the whole picture, and a business performing well here may perform differently elsewhere. Every figure comes from search results captured on the date stated and can be independently checked by running the same queries yourself.
Where Your Clinic Sits
This report shows the market. It does not show your position in it — whether you are cited at all, on which procedures, and whether Google is citing your website or only your listing. That analysis is specific to your practice, and we produce it individually.
With 16 clinics cited region-wide and three holding 53% of local citations, this is not a market where everyone is faintly visible — you are either quoted or you are absent. The useful part is how cleanly the work splits by procedure: breast augmentation is won with published surgical content, rhinoplasty and liposuction are won substantially through the Google Business Profile, and the two require entirely different effort.
