What did this study measure?

We ran 21 patient-intent cost queries through Google across seven Calgary-region sub-markets — rhinoplasty, breast augmentation and liposuction — and recorded every source cited by the AI Overview each one returned.

  • 356 citations captured, de-duplicated so a clinic cited twice in one answer counts once
  • 42% pointed at Google listings, 27% at clinics outside the region, 19% at Calgary-area clinic websites and 11% at non-providers
  • Only 13 distinct Calgary-area clinics were named — the smallest local field in the series
  • Canmore treated as outside the region and named explicitly rather than folded into either group
  • Single point in time — captured August 2, 2026, and generated answers vary between runs

This measures Google AI Overviews only. It does not measure ChatGPT, Perplexity, Gemini or Copilot, which use different retrieval systems and require separate tracking.

Four Findings

Calgary is losing its own market. 97 citations (27%) go to clinics with no Calgary-region location, against 69 (19%) to clinics that have one. By province: Ontario 37, Alberta outside the Calgary region 35, Quebec 16, outside Canada 5, other Canada 4. Every other vertical and city in this series has local providers ahead.

Four of the clinics reaching all seven sub-markets are not in Calgary. radianceplasticsurgery.ca and banffplasticsurgery.ca are both in Canmore, about 100 km west; royalcentreofplasticsurgery.com is in Barrie, Ontario; and two more are in Montreal. Radiance alone holds 17 citations — more than any Calgary clinic except one.

Liposuction is almost entirely a listing market. 61% of liposuction citations are Google Business Profile panels and only 9% reach a Calgary clinic's website. Rhinoplasty is the other end of the range: 28% local with 34% listings.

Google is citing sources that are not clinics at all. 39 citations (11%) went to a US surgeons' association, three patient-financing companies, two directories, a Miami luxury magazine — and, three times, a WordPress plugin-update endpoint on a British CBD company's domain. A Turkish medical-tourism hospital group takes another four.

Patients travel for a surgeon, so geography barely constrains the answer. What does change, sharply, is whether Google quotes a website or a listing.

Every domain below was opened and its published address recorded. None has a location in Calgary, Airdrie, Chestermere, Okotoks or Cochrane. radianceplasticsurgery.ca is in Canmore, about an hour and a half west of the city; it is cited in all seven Calgary sub-markets, on all three procedures, and holds more citations than every Calgary clinic but one.

Radiance is a real, reachable Alberta practice, so this is not an error on Google's part so much as a demonstration: a clinic 100 km outside a metro can own that metro's answers by publishing better. The same mechanism put Barrie, Montreal, Ottawa, Halifax, Langley, Ohio and Istanbul in Calgary's results.

In Okotoks and Chestermere, Outside Clinics Outnumber Local Ones Two to One

Calgary Central returned zero listing citations. Chestermere and Northeast Calgary returned 41 and 40. The listing lever and the content lever are worth completely different amounts depending on where the patient is searching from.

74 Businesses Named Through a Google Listing

151 citations (42%) point to a Google Business Profile panel rather than a website. These carry no publisher domain, so they are reported by business name. The list is dominated by individual surgeons and medical-aesthetics spas, several of which have no cited website anywhere in this study — Google knows them, but has nothing of theirs to quote.

The entry worth pausing on is plugin-update-check.britishcannabis.org: it is not a clinic, a directory or a publisher — it is a WordPress plugin-update endpoint on the domain of a British CBD company, and Google cited it three times as a source in Calgary cosmetic surgery cost answers.

Method

Proof, Not Promises

  1. STEP 01

    Ran 21 patient-intent cost queries through Google with city-level location targeting across seven Calgary-region sub-markets: Calgary Central, Northwest Calgary, Northeast Calgary, Southeast Calgary, Airdrie, Chestermere and Okotoks.

  2. STEP 02

    Tested each sub-market with the same three procedures — rhinoplasty, breast augmentation and liposuction, so every area was asked identical questions.

  3. STEP 03

    Captured the complete search result page for each query, including the AI Overview block, its generated text, and every source it cited.

  4. STEP 04

    De-duplicated citations per answer, so a clinic cited twice within a single AI Overview counts once.

  5. STEP 05

    Opened every cited provider domain individually and read its published address from the clinic's own contact page. Clinics with a verified location in the Calgary Metropolitan Region are counted as local; Canmore is treated as outside the region and named explicitly rather than folded into either group silently.

  6. STEP 06

    Counted Google Business Profile panels separately as listing citations, attributed to the business named in the panel, and classified associations, financing companies, directories, magazines and social platforms as non-provider sources.

  7. STEP 07

    Crawled on August 2, 2026 without personalisation, and recorded the crawl window so any figure can be re-checked against the day it was captured.

Limitations

This is a single-point-in-time snapshot. AI Overviews are generated fresh per query and vary between runs, so exact citation lists will differ if re-run; trends require repeated sampling. Clinic identification is by website domain, and where a group operates several sites each is counted separately unless the study states a merge.

Only three procedures were tested; facial, injectable and body procedures not covered here may behave differently. Several cited businesses are medical-aesthetics or dermatology clinics rather than surgical practices. Listing citations carry no publisher domain and are counted separately from website citations, so the two figures are not directly comparable. Sub-market labels follow Google's own location targeting, so “Northwest Calgary” is Google's quadrant rather than a municipal boundary.

AI Overviews reach the largest audience of any AI answer surface — no login, no app, shown to anyone searching Google — so this is the widest-reach measurement available. It is not the whole picture, and a business performing well here may perform differently elsewhere. Every figure comes from search results captured on the date stated and can be independently checked by running the same queries yourself.

Calgary cosmetic surgery is the only market in this series where out-of-region clinics out-cite local ones. Just 13 Calgary-area clinics are cited at all, and three of them hold 59% of local citations. The work splits cleanly: liposuction is won through the Google Business Profile, rhinoplasty and breast augmentation are won with published surgical content — and right now that content is being published in Canmore, Barrie and Montreal.
