# Who Google's AI Recommends for Physiotherapy in Vancouver

## Headline finding

**Metro Vancouver clinics hold 67% of their own market — the strongest local position we have measured anywhere.** The exception is the MSP coverage question, where half the answer goes to government, an insurer and a US digital-health company.

## What did this study measure?

We ran 21 patient-intent queries through Google across seven Metro Vancouver sub-markets and recorded every source cited by the AI Overview each one returned. The coverage question was localised to British Columbia — we asked about MSP rather than the Ontario or Alberta schemes used in our other studies.

- **20 of 21 queries** returned an AI Overview; the miss was cost per session in Richmond
- **216 citations captured**, de-duplicated so a clinic cited twice in one answer counts once
- **67% pointed at local clinic websites**, 19% at non-providers, 12% at Google listings and 2% at clinics outside the region
- **80 distinct Metro Vancouver clinics** were named — more than double Calgary or Ottawa
- **Multidisciplinary clinics counted as clinics** where they offer physiotherapy among other services
- **Single point in time** — captured August 2, 2026, and generated answers vary between runs

## Findings

### This is the best-defended market in the series

144 of 216 citations (67%) go to a clinic with a Metro Vancouver address, and only 5 (2%) go to clinics outside the region — two clinics in total, one in Abbotsford and one in Scarborough, Ontario. Calgary physiotherapy leaked 20% to Edmonton and Grande Prairie; Vancouver leaks almost nothing.

### MSP coverage is the one question clinics lose

Local clinics take 76% of car-accident citations and 65% of cost-per-session — but only 51% of “is physiotherapy covered by MSP”. 49% of that question goes to non-providers: the Government of BC, Canada Life, Reddit and Sword Health, a digital-health company that operates no clinics and publishes no address.

### ICBC is a competitor, not just a payer

ICBC's own domains take 6 citations on the car-accident question. In Ontario and Alberta that role is played by law firms and private insurers; in British Columbia the public auto insurer answers the question directly, and it does so better than most clinics.

### The field is enormous and flat

80 distinct Metro Vancouver clinics were cited. 44 of them (55%) appear exactly once, the top ten hold 33% of local citations, and only 4 clinics answer all three patient questions.

Patients ask three different things and the competitor changes each time: other clinics on cost, the province and an insurer on coverage, ICBC on car accidents.

## Two Clinics, Five Citations, 2% of the Market

The entire out-of-region table for this study fits in two rows. That is the smallest intrusion we have measured in any vertical or metro, and it is why Vancouver's remaining gaps are unusually specific: the competition is not another city, it is the province, an insurer and a software company.

## Every Metro Vancouver Clinic Cited, Ranked

All 80 clinics cited across the 21 queries analysed, ranked by how many generated answers cited the clinic's own website. No clinic holds more than 8 citations, and only four — reverephysio.com, prosperhealth.ca, launchrehab.ca and northvancouverphysio.ca — answer all three patient questions.

## Richmond Is the Weakest Sub-Market

One of Richmond's three queries returned no AI Overview at all, and it produced the fewest distinct clinics. Burnaby is the only sub-market where Google listings feature materially — 23 of the study's 25 listing citations appeared there.

## Government, ICBC and a Digital-Health Company With No Clinics

42 citations (19%) went to sources that are not Metro Vancouver clinics. swordhealth.com is Sword Health, a digital musculoskeletal-care company. It operates no clinic in British Columbia — or anywhere — and publishes no street address on its site. It is cited 5 times on the MSP coverage question, more than all but three Metro Vancouver clinics manage on that question.

## Three Cities, Three Different Games

Ottawa is a listing market. Calgary is a pure content market that leaks a fifth of its citations to Edmonton. Vancouver is a content market that local clinics already dominate — which is what makes the remaining gaps unusually specific.

## Method

1. Ran 21 patient-intent queries through Google with city-level location targeting across seven Metro Vancouver sub-markets: Vancouver, Burnaby, Richmond, Surrey, North Vancouver, Coquitlam and Langley.
2. Tested each sub-market with the same three questions — cost per session, MSP coverage, and treatment after a car accident, so every area was asked identical questions.
3. Captured the complete search result page for each query, including the AI Overview block, its generated text, and every source it cited.
4. De-duplicated citations per answer, so a clinic cited twice within a single AI Overview counts once.
5. Opened every cited provider domain whose name did not already carry an unambiguous Metro Vancouver place name and read its published address from the clinic's own contact page; Abbotsford is treated as outside Metro Vancouver and named explicitly.
6. Counted government, ICBC, insurers, directories, booking platforms, digital-health companies and a university teaching clinic as non-provider sources, and counted multidisciplinary clinics offering physiotherapy among other services as clinics.
7. Crawled on August 2, 2026 without personalisation, and recorded the crawl window so any figure can be re-checked against the day it was captured.

## Limitations

This is a single-point-in-time snapshot. AI Overviews are generated fresh per query and vary between runs, so exact citation lists will differ if re-run; trends require repeated sampling. Clinic identification is by website domain, and where a group operates several sites each is counted separately unless the study states a merge.

One of the 21 queries — cost per session in Richmond — returned no AI Overview and contributes no citations. Only three patient questions were tested; pelvic health, vestibular, concussion and post-surgical rehabilitation may behave differently. Sub-market labels follow Google's own location targeting rather than municipal boundaries.

AI Overviews reach the largest audience of any AI answer surface — no login, no app, shown to anyone searching Google — so this is the widest-reach measurement available. It is not the whole picture, and a business performing well here may perform differently elsewhere. Every figure comes from search results captured on the date stated and can be independently checked by running the same queries yourself.

## Where your clinic sits

This report shows the market. It does not show your position in it — whether you are cited at all, on which questions, and which clinic is being quoted in your place. That analysis is specific to your clinic, and we produce it individually.

Vancouver physiotherapy is unusual in this series because the competitor is not another city — it is the provincial government, ICBC and a US software company. Those three cannot be out-competed on service, only on publishing.

And the specific gap is narrow enough to name: MSP coverage, where no listing citations exist at all, 49% of the answer belongs to non-providers, and four clinics currently hold the rest. ## Questions about this report

### Why does this report only cover Google AI Overviews?

Because an AI Overview is rendered inside a public search results page, it can be retrieved directly, for a specific city, at a specific moment — which is how every figure here was captured. ChatGPT, Perplexity, Gemini and Copilot do not publish their answers anywhere; those answers exist only when a user prompts the model, so measuring them requires running a fixed prompt set against each model repeatedly over time. That is a different instrument, and we report it separately rather than blending two methods into one number.

### Would you get the same numbers if you ran it again?

Not exactly. AI Overviews are generated fresh per query and vary between runs, so the exact citation list would differ. The structural findings — how concentrated the market is, which source types dominate, which questions are being lost — are stable enough to act on. Any specific clinic's citation count is a snapshot, not a ranking position.

### Our clinic is not in the table. What does that mean?

It means the clinic was not cited in any of the 20 answers Google generated, in any of the seven sub-markets. 80 clinics were cited and 44 of them exactly once, so this is the widest field in the series — absence is common, and the gap between absent and cited is often one page. It does not mean the clinic is invisible in Google generally.

### If local clinics already hold 67%, is there anything to do?

On two of the three questions, less than you might think — car-accident rehab is at 76% and cost per session at 65%, and the remainder is mostly Google listings and ICBC. The MSP coverage question is the exception, and it is where the whole opportunity sits: 49% of it goes to non-providers, no listing citation exists to compete with, and four clinics currently hold what is left. A strong market average can hide a completely unclaimed question.

### How were clinics identified?

By website domain, exactly as it appeared in the AI Overview source list. Where a group operates several sites, each is counted separately unless this study states otherwise, because the citation was earned by a specific page on a specific domain.

### Can we be removed from the report?

Write to us and we will discuss it. The domains here appear because they were cited in publicly generated search results on the date stated, and the study would be less useful and less checkable without them — but we would rather have the conversation than not.

### Does being cited more often mean more customers?

We did not measure that, and we will not claim it. This study measures citations in generated answers, not clicks, calls or bookings. What it establishes is which clinics are present in the answer someone reads before choosing — presence is a precondition for being chosen, not a substitute for it.
