# Who Google's AI Recommends for Cosmetic Surgery in Vancouver

> A citation study of Google AI Overviews across 7 Metro Vancouver sub-markets, measuring which clinics AI-generated answers name and cite. 16 clinics, 315 citations, 21 queries, captured August 2, 2026.

A citation study of Google AI Overviews across seven Metro Vancouver sub-markets and three procedures — rhinoplasty, breast augmentation and liposuction — measuring which clinics AI-generated answers name and cite.

21 patient-intent cost queries. 315 citations captured. Only 16 Metro Vancouver clinics cited across the whole region, and three of them hold 53% of local citations.

## Headline finding

**Only 16 Metro Vancouver clinics are cited across the whole region, and three of them hold 53% of local citations.** A clinic in Barrie, Ontario is cited in all seven sub-markets — and one of the intruders is in Vancouver, Washington.

| Figure | Value |
| --- | --- |
| Metro Vancouver clinics cited | 16 |
| Citations to local clinics | 30% |
| Citations to Google listings | 41% |
| Citations to clinics outside the region | 20% |

## What did this study measure?

We ran 21 patient-intent cost queries through Google across seven Metro Vancouver sub-markets — rhinoplasty, breast augmentation and liposuction — and recorded every source cited by the AI Overview each one returned.

- **21 of 21 queries** returned an AI Overview — every cost question we asked
- **315 citations captured**, de-duplicated so a clinic cited twice in one answer counts once
- **41% pointed at Google listings**, 30% at local clinic websites, 20% at clinics outside the region and 9% at non-providers
- **Only 16 distinct Metro Vancouver clinics** were named across the entire region
- **Two consolidations applied** where several domains resolve to one practice at one published address
- **Single point in time** — captured August 2, 2026, and generated answers vary between runs

_This measures Google AI Overviews only. It does not measure ChatGPT, Perplexity, Gemini or Copilot, which use different retrieval systems and require separate tracking._

## Findings

### The local field is tiny and top-heavy

Just 16 Metro Vancouver clinics were cited across the entire region, and the top three hold 53% of local citations. Three of those — Pacific Plastic Surgery Group, Dr. Colin McInnes and YES — appear in all seven sub-markets. Below them the market thins out fast.

### An Ontario clinic is the second most-cited website in Vancouver

royalcentreofplasticsurgery.com, in Barrie, Ontario, takes 18 citations across all seven sub-markets and all three procedures — more than every Metro Vancouver clinic except one. In total 20% of citations go to clinics with no Metro Vancouver location: Barrie, Montreal, Toronto, Mississauga, Ottawa, Abbotsford, Nanaimo, Ohio, Virginia — and Vancouver, Washington.

### Rhinoplasty and liposuction are listing markets; breast augmentation is not

50% of rhinoplasty citations and 51% of liposuction citations are Google Business Profile panels. Breast augmentation returns just 3% listings — and it is the one procedure where local clinics lead, at 56%.

### Vancouver proper returned no listing citations at all

Zero, across all three procedures. Surrey returned 40, Burnaby 22, Richmond 22, Coquitlam 21 and Langley 20. A Business Profile is worth nothing downtown and a great deal everywhere else.

## Three Procedures, Three Different Markets

Patients travel for a surgeon, so geography barely constrains the answer. What does change, sharply, is whether Google quotes a website or a listing — and this is the sharpest split of the three cities we have measured.

_Share of citations going to a Metro Vancouver clinic's own website_

| Topic | Citations | Local share | Most cited | Note |
| --- | --- | --- | --- | --- |
| Breast augmentation | 66 | 56% | yescosmeticsurgery.com (7), doctorm.ca (7), pacificplasticsurgerygroup.ca (7) | Two listing citations in the whole procedure. Content wins outright, and local clinics lead — though 26% still goes to clinics outside the region. |
| Rhinoplasty | 126 | 27% | doctordenton.com (6), pacificplasticsurgerygroup.ca (6), youngerface.com (5) | Half of this answer is Google Business Profile panels. A complete, well-maintained listing does more here than another page of copy. |
| Liposuction | 123 | 20% | doctorm.ca (7), yescosmeticsurgery.com (7), pacificplasticsurgerygroup.ca (6) | 51% listings and the lowest local website share of the three. Optimising the wrong asset for the procedure you care about is wasted effort. |

## 17 Clinics, 62 Citations, 20% of the Market

Every domain below was opened and its published address recorded. None has a location in Metro Vancouver. salmoncreekps.com is Salmon Creek Plastic Surgery of Vancouver, Washington — about 500 km south, in a different country. It was cited in two Metro Vancouver rhinoplasty answers. Google's summariser matched the city name and not the jurisdiction; it is the same failure mode we found in Vancouver dentistry, where a Richmond Hill, Ontario practice was cited for Richmond, BC.

## All 16 Metro Vancouver Clinics Cited

The full list is short enough to print in its entirety — which is itself the finding. Ranked by how many generated answers cited the clinic's own website. Three clinics were cited across all three procedures.

## Vancouver Returned Zero Listing Citations; Surrey Returned Forty

In Burnaby and Richmond, out-of-region clinics are close to level with local ones. Downtown, they are not — and downtown is also the only sub-market where a Google Business Profile earns nothing at all.

| Sub-market | Calgary clinic citations | Out of region | Listing citations | Distinct Metro Vancouver clinics |
| --- | --- | --- | --- | --- |
| Vancouver | 11 | 7 | 0 | 9 |
| Burnaby | 12 | 10 | 22 | 8 |
| Richmond | 13 | 12 | 22 | 9 |
| Surrey | 15 | 9 | 40 | 9 |
| North Vancouver | 14 | 10 | 3 | 9 |

## 82 Businesses Named Through a Google Listing

128 citations (41%) point to a Google Business Profile panel rather than a website. These carry no publisher domain, so they are reported by business name. The list is dominated by individual surgeons and medical-aesthetics clinics, many of which have no cited website anywhere in this study — Google knows them, but has nothing of theirs to quote.

## Financing Companies, Medical Tourism and a Deals Marketplace

28 citations (9%) went to sources that are not clinics: three patient-financing companies, two medical-tourism platforms — one based in Kyiv — a US surgeons' association, a deals marketplace and an insurer. Financing companies answer the cost question directly, which is exactly what the query asks.

| Source | What it is | Citations |
| --- | --- | --- |
| ifinancecanada.com | Patient financing company (Toronto HQ; Vancouver office) | 7 |
| plasticsurgery.org | US professional association (ASPS) | 4 |
| beautifi.com | Patient financing company | 3 |
| youtube.com | Video platform | 2 |
| instagram.com | Social platform | 2 |
| reddit.com | Community forum | 2 |
| groupon.com | Deals marketplace (Chicago) | 2 |
| canadalife.com | Insurer | 2 |
| us-uk.bookimed.com | Medical-tourism platform (Kyiv, Ukraine) | 1 |
| yapitahealth.com | Medical-tourism platform (no address published) | 1 |
| carecredit.com | US patient financing (Synchrony Bank) | 1 |
| whatclinic.com | Global clinic directory (Dublin, Ireland) | 1 |

## Method

1. Ran 21 patient-intent cost queries through Google with city-level location targeting across seven Metro Vancouver sub-markets: Vancouver, Burnaby, Richmond, Surrey, North Vancouver, Coquitlam and Langley.
2. Tested each sub-market with the same three procedures — rhinoplasty, breast augmentation and liposuction, so every area was asked identical questions.
3. Captured the complete search result page for each query, including the AI Overview block, its generated text, and every source it cited.
4. De-duplicated citations per answer, so a clinic cited twice within a single AI Overview counts once.
5. Opened every cited provider domain individually and read its published address from the clinic's own contact page. Abbotsford and Nanaimo are treated as outside Metro Vancouver and named explicitly; where several domains resolve to a single practice at one published address they are counted once and the consolidation is stated.
6. Counted Google Business Profile panels separately as listing citations, attributed to the business named in the panel, and classified financing companies, associations, directories, medical-tourism platforms, insurers and social platforms as non-provider sources.
7. Crawled on August 2, 2026 without personalisation, and recorded the crawl window so any figure can be re-checked against the day it was captured.

## Limitations

This is a single-point-in-time snapshot. AI Overviews are generated fresh per query and vary between runs, so exact citation lists will differ if re-run; trends require repeated sampling. Clinic identification is by website domain, and where a group operates several sites each is counted separately unless the study states a merge.

Only three procedures were tested; facial, injectable and body procedures not covered here may behave differently. Several cited businesses are medical-aesthetics clinics rather than surgical practices. Two domains block automated retrieval and are reported as unverified rather than assigned to a region. Listing citations carry no publisher domain and are counted separately from website citations, so the two figures are not directly comparable. Sub-market labels follow Google's own location targeting rather than municipal boundaries.

AI Overviews reach the largest audience of any AI answer surface — no login, no app, shown to anyone searching Google — so this is the widest-reach measurement available. It is not the whole picture, and a business performing well here may perform differently elsewhere. Every figure comes from search results captured on the date stated and can be independently checked by running the same queries yourself.

## Where your clinic sits

This report shows the market. It does not show your position in it — whether you are cited at all, on which procedures, and whether Google is citing your website or only your listing. That analysis is specific to your practice, and we produce it individually.

With 16 clinics cited region-wide and three holding 53% of local citations, this is not a market where everyone is faintly visible — you are either quoted or you are absent. The useful part is how cleanly the work splits by procedure: breast augmentation is won with published surgical content, rhinoplasty and liposuction are won substantially through the Google Business Profile, and the two require entirely different effort.

The work that follows from this report is a fixed-price engagement: [the AI Visibility Accelerator](https://enginely.ai/services/ai-visibility-accelerator/index.html.md). Thirteen weeks, a published price, and three separate measurements against these same queries after publication.

## Questions about this report

### Why does this report only cover Google AI Overviews?

Because an AI Overview is rendered inside a public search results page, it can be retrieved directly, for a specific city, at a specific moment — which is how every figure here was captured. ChatGPT, Perplexity, Gemini and Copilot do not publish their answers anywhere; those answers exist only when a user prompts the model, so measuring them requires running a fixed prompt set against each model repeatedly over time. That is a different instrument, and we report it separately rather than blending two methods into one number.

### Would you get the same numbers if you ran it again?

Not exactly. AI Overviews are generated fresh per query and vary between runs, so the exact citation list would differ. The structural findings — how concentrated the market is, which source types dominate, which procedures are being lost — are stable enough to act on. Any specific clinic's citation count is a snapshot, not a ranking position.

### Our clinic is not in the table. What does that mean?

It means the clinic's website was not cited in any of the 21 answers measured. Only 16 Metro Vancouver clinics were, so absence is overwhelmingly the normal case here. It does not mean the clinic is invisible in Google generally — 82 businesses were named through listing panels, many with no cited website anywhere in this study, and yours may be among them.

### Why were some domains merged into a single clinic?

Because a prospect clicking any of them reaches the same practice at the same address. Three domains resolve to one clinic at 555 West 8th Avenue in Vancouver, and three more to one clinic at 13737 96 Avenue in Surrey. Counting them separately would inflate the local field from 16 clinics to 20 and make the market look less concentrated than it is. The consolidation is stated rather than applied quietly.

### How were clinics identified?

By website domain, exactly as it appeared in the AI Overview source list. Where a group operates several sites, each is counted separately unless this study states otherwise, because the citation was earned by a specific page on a specific domain.

### Can we be removed from the report?

Write to us and we will discuss it. The domains here appear because they were cited in publicly generated search results on the date stated, and the study would be less useful and less checkable without them — but we would rather have the conversation than not.

### Does being cited more often mean more customers?

We did not measure that, and we will not claim it. This study measures citations in generated answers, not clicks, calls or bookings. What it establishes is which clinics are present in the answer someone reads before choosing — presence is a precondition for being chosen, not a substitute for it.

## Legal note

This report is provided for information. Clinic and business names appear because they were cited in publicly generated search results on the date stated; inclusion or absence is not an assessment of surgical skill, clinical quality or standing. Nothing here is medical advice. Clinic locations were read from each clinic’s own published website on August 2, 2026 and may change. Figures describe what Google's systems returned on August 2, 2026 and may change.
