# Who Google's AI Recommends for Physiotherapy in the GTA

## Headline finding

**Only 3 of the 66 clinics cited were named across all three patient questions we tested.** Almost every clinic in the GTA wins one question and is invisible on the other two.

## What did this study measure?

We asked Google the three questions physiotherapy patients actually ask — what a session costs, whether OHIP covers it, and what happens after a car accident — across seven GTA sub-markets, and recorded every source cited by the AI Overview each one returned.

- **18 of 21 queries** returned an AI Overview; three showed a map pack instead
- **184 citations captured**, de-duplicated so a clinic cited twice in one answer counts once
- **77% pointed at clinic websites** — the highest share of the three verticals we have measured
- **66 distinct clinics** named across the seven sub-markets
- **Three patient questions**, each answered from a different kind of page
- **Only 3 clinics** were cited on all three

## Findings

### Clinics own these answers — more than dentists or lawyers do

77% of all citations went to a clinic's own website rather than a directory, insurer or government page. That is the highest share of the three verticals we have measured. What you publish is what gets quoted.

### But which question you win matters enormously

Clinics take 88% of citations on car-accident rehab and 79% on session cost — but only 49% on OHIP coverage, where the College of Physiotherapists, the Ontario government and insurers take the rest.

### Almost nobody wins more than one question

Just 3 of 66 clinics were cited across all three. The rest appear on one topic and vanish on the others, which means most clinics are invisible for two-thirds of the questions their patients ask.

### The leaders are pulling away

The ten most-cited clinics hold 43% of all citations, and 19 clinics appear in more than one sub-market — two of them in all seven. This is more concentrated than either the dental or legal markets we have measured.

## Where Clinics Win, and Where They Lose

This is the most useful pattern in the data. The same clinic can dominate one patient question and be entirely absent from another, because each question is answered from a different kind of page.

## The OHIP Gap

OHIP coverage is the weakest position for clinics in this study and the clearest opportunity. Barely half those citations go to a clinic. The rest are taken by collegept.org — the College of Physiotherapists of Ontario — by ontario.ca, and by insurers such as Canada Life.

Patients asking this question are trying to work out whether they can afford treatment at all. Most clinics have published nothing that answers it.

Car-accident rehab is the opposite case: clinics take 88% of citations there, and it is the highest-value referral stream in the field because treatment is insurer-funded. It is also the question where the most clinics are competing.

All 66 physiotherapy clinics cited in GTA AI Overviews, ranked by how many generated answers cited the clinic's own website. The “missing” column is the more useful one: it names the patient questions each clinic was absent from.

## Not Every Part of the GTA Has AI Answers Yet

Three of the 21 queries returned no AI Overview at all — Google showed a map pack instead. All three were in Brampton and Markham. This is worth watching rather than dismissing: where the AI Overview has not yet appeared, the clinic that publishes the answer first is likely to be the one cited when it does.

## Directories, Insurers and One Silicon Valley Platform

Of 184 citations captured, 142 (77%) went to a clinic's own website. The remainder is worth knowing, because it shows exactly who is standing between a clinic and the patient.

_These 13 sources account for 36 of the 42 non-clinic citations; the study did not itemise the remainder. Sword Health is worth singling out: a US digital musculoskeletal platform with no GTA clinic, cited four times in answers about Toronto-area physiotherapy. Booking directories such as PainHero and HealthHives take citations on the same basis — they publish clear, structured answers about cost and coverage, and clinics largely do not._

## Method

1. Ran 21 patient-intent queries through Google with city-level location targeting across seven GTA sub-markets: Toronto (Downtown), North York, Scarborough, Mississauga, Brampton, Markham and Vaughan.
2. Tested each sub-market with the same three questions — session cost, OHIP coverage, and cost of treatment after a car accident.
3. Captured the complete search result page for each query, including the AI Overview block, its generated text, and every source it cited.
4. Crawled across 30–31 July 2026, without personalisation.
5. De-duplicated citations per answer. Where a clinic's booking page on a third-party scheduling platform was cited, it was attributed to that clinic.
6. Classified each cited domain by hand. Community health teams and hospital-affiliated programmes were counted as non-clinic sources, because they are not private practices.

## Limitations

This is a single-point-in-time snapshot. AI Overviews are generated fresh per query and vary between runs, so exact citation lists will differ if re-run; trends require repeated sampling. Clinic identification is by website domain, and where a group operates several sites each is counted separately.

Three queries returned no AI Overview and contribute no citations to any figure here. Community health teams and hospital-affiliated programmes were counted as non-clinic sources.

Every figure here comes from search results captured on the dates stated and can be independently checked by running the same queries yourself. We publish the method so the numbers can be tested, not taken on trust.

## What This Report Does Not Show

It does not show your clinic's position — your citation count, which of the three patient questions you are absent from, and which clinics are named in the answers your patients see instead.

The pattern in this data is unusually specific about what to fix. With only 3 of 66 clinics cited across all three questions, and OHIP coverage answered by clinics barely half the time, the gap for most practices is not general visibility. It is one or two missing pages.

## Questions about this report

### Why is OHIP coverage the weakest question for clinics?

Because most clinics have published nothing that answers it. Barely half those citations reach a clinic — the rest go to collegept.org, ontario.ca and insurers such as Canada Life, all of whom have clear, structured pages explaining what is and is not covered. Patients asking this question are working out whether they can afford treatment at all, which makes it the cheapest gap in this study to close.

### Three queries returned no AI Overview. Does that weaken the study?

It narrows it, and we report it rather than dropping those queries quietly. All three misses were in Brampton and Markham, where Google showed a map pack instead. Those three contribute no citations to any figure here. It is worth watching rather than dismissing: where the AI Overview has not yet appeared, the clinic that publishes the answer first is likely to be the one cited when it does.

### Why is a US digital platform cited in Toronto answers?

Sword Health is a US digital musculoskeletal platform with no GTA clinic, and it was cited four times in answers to questions asked about Toronto-area physiotherapy. It earned those citations the same way the booking directories did — by publishing clear, structured answers about cost and coverage that local clinics largely have not. An AI Overview has no proximity requirement.

### Our clinic is not in the table. What does that mean?

It means the clinic was not cited in any of the 18 answers measured, in any of the seven sub-markets. It does not mean the clinic is invisible in Google generally, or that it performs badly in the map pack — those are different surfaces measured differently. It does mean that for these three questions, the answer a patient received named someone else.

### Why does this report only cover Google AI Overviews?

Because an AI Overview is rendered inside a public search results page, it can be retrieved directly, for a specific city, at a specific moment. ChatGPT, Perplexity, Gemini and Copilot do not publish their answers anywhere; those answers exist only when a user prompts the model, so measuring them requires running a fixed prompt set against each model repeatedly over time. That is a different instrument, and we report it separately rather than blending two methods into one number.

### Can we be removed from the report?

Write to us and we will discuss it. The domains here appear because they were cited in publicly generated search results on the dates stated, and the study would be less useful and less checkable without them — but we would rather have the conversation than not.
