# Who Google's AI Recommends for Cosmetic Surgery in the GTA

## Headline finding

**Just 42 clinic websites account for every AI Overview citation across the entire Greater Toronto Area, and ten of them hold 55% of those citations.** This is the most closed market we have measured.

## What did this study measure?

We asked Google what three cosmetic procedures cost — rhinoplasty, breast augmentation and liposuction — across seven GTA sub-markets, and recorded every source cited by the AI Overview each one returned, separating clinic websites from Google Business Profile listings.

- **21 of 21 queries** returned an AI Overview — no negative cases
- **294 citations captured**, split 167 websites / 109 listings / 18 third party
- **Only 42 clinic websites** — by far the smallest field of any vertical we have measured
- **85 distinct Google listings**, a much wider and entirely separate pool
- **Just 6% to third parties** — no aggregator problem in this market
- **Single point in time** — captured 30–31 July 2026, and generated answers vary between runs

## Findings

### Every query produced an AI Overview

All 21 returned a generated answer naming clinics — across all three procedures and all seven sub-markets, with no exceptions.

### The market is close to closed

Only 42 clinic websites were cited in the entire GTA. The ten strongest hold 55% of citations, and just 26% of cited clinics appeared only once. For comparison, in dentistry 76% of cited practices appeared exactly once.

### Geography barely exists here

29 of the 42 cited clinics appear in more than one sub-market, and 5 appear in all seven. Patients travel for a surgeon, and the AI answers reflect that.

### Over a third of citations are not websites at all

37% point to Google Business Profile listings rather than a clinic's site — a pattern we have not seen in any other vertical. For liposuction it is the majority of citations.

## For Some Procedures, Your Google Listing Outranks Your Site

This is the structural surprise in the data. In dentistry, law, physiotherapy and accounting, AI Overviews cite provider websites almost exclusively. In cosmetic surgery a large share of citations instead point to Google Business Profile panels — the listing, not the site — and the proportion swings dramatically by procedure.

## Optimising the Wrong Asset Is Wasted Effort

For breast augmentation, essentially every citation is a website. Content wins outright, and a Google Business Profile earns nothing on that procedure.

For liposuction — the highest-volume procedure measured — the majority are listings. A complete, well-maintained profile does more there than another page of copy.

Rhinoplasty sits between the two at roughly two thirds website, one third listing, so both assets are doing work. The practical consequence is that there is no single right answer for a cosmetic surgery practice: the asset worth investing in depends on the procedure you actually want more of.

All 42 clinic websites cited in GTA AI Overviews across the 21 queries analysed, ranked by how many generated answers cited the clinic's own domain. This is the closed pool — 42 sites account for every website citation in the entire metro.

## Every Google Business Profile Cited, Ranked

A separate and much wider pool: 85 businesses were cited through their Google Business Profile panel rather than a website. None of them also appears in the website leaderboard above — the two pools are entirely disjoint, which makes a listing the easier entry point into these answers.

## Listing Citations Cluster in the 905

Listing citations are absent from Toronto and North York, where website citations dominate instead, and heaviest in Vaughan. We do not have an explanation for the split and are not going to guess at one — but if your clinic is in the 905, your Google Business Profile is carrying more of the answer than it would downtown.

## Almost Nobody — and That Matters

Only 18 of 294 citations (6%) went to a third party rather than a clinic website or listing. Medical tourism marketplaces, financing companies and directories are present but marginal.

_This is unusual and worth understanding. In several adjacent markets, referral aggregators and comparison sites take a large share of AI citations, leaving providers competing against companies whose entire business is ranking. Cosmetic surgery does not have that problem. The answers belong to clinics — which means the constraint is not intermediaries. It is that so few clinics have published anything worth citing._

These clinics are not competing for a neighbourhood. They are the default answer across the metro.

## Method

1. Ran 21 patient-intent cost queries through Google with city-level location targeting across seven GTA sub-markets: Toronto (Downtown), North York, Scarborough, Mississauga, Brampton, Markham and Vaughan.
2. Tested each sub-market with the same three procedures — rhinoplasty, breast augmentation and liposuction.
3. Captured the complete search result page for each query, including the AI Overview block, its generated text, and every source it cited.
4. Crawled across 30–31 July 2026, without personalisation. Citations were de-duplicated per answer.
5. Counted sources resolving to a clinic's own domain as website citations. Sources resolving to a Google Business Profile panel were counted separately as listing citations and attributed to the business named in the panel.
6. Counted everything else — directories, marketplaces, financing, associations, video and forums — as third party.

## Limitations

This is a single-point-in-time snapshot. AI Overviews are generated fresh per query and vary between runs, so exact citation lists will differ if re-run; trends require repeated sampling. Clinic identification is by website domain, and where a group operates several sites each is counted separately.

Listing citations carry no publisher domain and are reported by the business name shown in the Google panel. Where a business operates under more than one name, or a panel name differs from its trading name, that attribution is only as good as the panel.

Only three procedures were tested. Facial and body procedures not covered here may behave differently — and given how sharply the listing share swings across the three we did measure, we would expect variation rather than assume it away.

Every figure here comes from search results captured on the dates stated and can be independently checked by running the same queries yourself. We publish the method so the numbers can be tested, not taken on trust.

## What This Report Does Not Show

It does not show your clinic's position — whether you are cited at all, on which procedures, and whether Google is citing your website or only your listing.

The pattern here is unlike the other markets we measure. With only 42 clinic websites cited across the entire GTA and the top ten holding 55% of citations, this is not a market where everyone is faintly visible. Either you are one of the clinics Google quotes, or you are absent from the answer entirely — and today most clinics are absent.

## Questions about this report

### Why are Google listings cited here but not in your other studies?

We do not know why Google treats this vertical differently, and we will not invent a reason. What we can report is that it does: 37% of citations in cosmetic surgery point at a Google Business Profile panel rather than a website, against effectively none in the dental, legal, physiotherapy and accounting studies we ran with the same method. The share also swings hard by procedure — 61% on liposuction, zero on breast augmentation.

### Should we work on our website or our Google Business Profile?

It depends entirely on the procedure you care about, which is the practical value of this study. For breast augmentation every citation we captured was a website, so a Business Profile earns nothing there and content is the only lever. For liposuction the majority were listings, so a complete, well-maintained profile does more than another page of copy. Optimising the wrong asset for your priority procedure is wasted effort.

### Why is the listings table larger than the website table?

Because 85 distinct businesses appeared as listing citations against only 42 as websites, and the two pools do not overlap at all — no business cited via its listing also appeared as a website in this capture. ### Our clinic is not in either table. What does that mean?

It means the clinic was not cited in any of the 21 answers measured, in any of the seven sub-markets, as either a website or a listing. It does not mean the clinic is invisible in Google generally, or that it performs badly in the map pack — those are different surfaces measured differently. In this market that absence is common: most GTA clinics are not in the answer at all.

### Only three procedures were tested. What about facial and body work?

Not measured, and we would not extrapolate. Rhinoplasty, breast augmentation and liposuction were chosen as the highest-volume procedures; facial and body procedures not covered here may behave differently, and given how sharply the listing share swings between the three we did test, we would expect variation rather than assume it away.

### Why does this report only cover Google AI Overviews?

Because an AI Overview is rendered inside a public search results page, it can be retrieved directly, for a specific city, at a specific moment. ChatGPT, Perplexity, Gemini and Copilot do not publish their answers anywhere; those answers exist only when a user prompts the model, so measuring them requires running a fixed prompt set against each model repeatedly over time. That is a different instrument, and we report it separately rather than blending two methods into one number.

### Can we be removed from the report?

Write to us and we will discuss it. The names here appear because they were cited in publicly generated search results on the dates stated, and the study would be less useful and less checkable without them — but we would rather have the conversation than not.
